2011 Baseball Post Season Challenge Entry Form

The deadline for entering is midnight PST Saturday, June 11th, 2011.

Note...This challenge is open only to persons with a United States mailing address.

Entries posted after June 11th are VOID and will not be accepted.

Form Instructions

Entry Errors

We do some immediate validation of the information on the submitted form. If we find a problem, a message will be presented defining the problem. You can then make any required changes validate and resubmit.

Multiple Entries

There is an entry counter just above the validate button. Indicate in this counter if you are entering multiple times. The total entry fee and payment method for all entries should be indicated with your first entry. As each entry is submitted an acknowledgement message will be presented and the counter will be advanced. You can then make any desired changes in player selections in the form below the message validate and submit your next entry.

Previous Contestants

If you have been a previous contestant with Sports Fanatics Player Drafts please check the previous contestant box. If you know your contestant ID from a previous report, enter it in the ID boxes.

If you have a change of address, please check the change of address box and enter the new address information. In this instance your contestant ID number would be helpful.

Email Address

The Email address is optional. However, if you provide an Email address we will use it to acknowledge receipt of your entry and your payment. We may also use it to inform you of any future contests. You will always get our regular mailing for this contest via the US postal service.

Entry Fee

The challenge entry fee in the US is $20 for one entry, and only $10 for each additional entry. This reduced multiple entry rate is per contestant not per address.

We have a secure server. If you are using an SSL secure browser ( Internet Explorer, Firefox or Netscape Navigator 2.0 and later) you can send us your credit card info via this entry form. If you are not using a secure browser, we do not encourage contestants to send credit card info via this entry form. We therefore provide several alternate methods of paying the entry fee. Please check the method you prefer. We can accept only VISA, MASTERCARD and DISCOVER credit cards.

If you do not provide a credit number, when paying for your entry, please refer to your name and ID number and or address which will associate your payment with your entry. Your entry fee must be received by June 16th . You will not be officially entered into the challenge until we receive your credit card number, check or money order.
